The survey will take about 15-20 minutes to complete.

Your answers will remain confidential. The Strategic Counsel will report only the combined results of all those who complete the survey.

You may wish to have your insurance documentation close by when you fill out the survey so that you can refer to the documents in order to answer some of the survey questions.
